MAKL – MOUNTAINEER ADULT KICKBALL LEAGUE
WAIVER OF LIABILITY AND ASSUMPTION OF RISK
By submitting this form, I acknowledge that participating in the MAKL (Mountaineer Adult Kickball League) athletic sports program carries a risk of personal injury.
I acknowledge that I must sign a player liability waiver in order to participate in the Mountaineer Adult Kickball League (MAKL).
The player's waiver and release will act to RELEASE and DISCHARGE MAKL and BOPARC's employees, volunteers, agents, and staff from liability for injuries or damages, including any losses caused by negligent, reckless, willful, or wrongful acts or omissions of participants.
Participants agree not to sue or make any claims against MAKL, Tessa Kimerer, BOPARC, or any of their elected officials, officers, employees, agents, or volunteers. Participants further agree to hold them harmless and indemnify them against all claims and losses that may result from use of facilities or participation in sports and recreational programs.
By signing this form, I agree to honor MAKL regulations and the MAKL Code of Conduct.
I understand that if I decline to execute this agreement, I will not be permitted to participate in this league or any games.
I confirm that I have read and understand the MAKL Official Rulebook and the code of conduct that players must follow during MAKL games, related events, and activities.
Participants under the age of 18 at the time of registration are not eligible to participate in MAKL.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
PHOTO RELEASE
MAKL reserves the right to post photographs of official MAKL events online and in print media.
By submitting this form and checking the agreement boxes, I confirm that this acts as my legal electronic signature acknowledging and agreeing to the MAKL waiver of liability and photo release.
